Shift Differentials:   Standard Rates

Overtime Rate:   1.5 x standard rate for any hours over 40 worked in a week in accordance with state labor laws.

Federal Holiday Rate:   1.5 x standard rate for all Government-recognized Federal Holidays.

On-Call and Call-Back Rates and Terms:   On-Call rate is $4.00 per hour in which provider is on-call status. Minimum two (2) hours billed. On-call provider is to be available to report to the facility within 30 minutes or provide telephone consultation. If provider, while on call, is required to return to facility, base rates and terms apply if less than 40 hours are worked in a week or overtime rates and terms apply if more than 40 hours are worked in a week.

Government Work Week:   12:01 AM Sunday through midnight Saturday.

Maximum Order:   The maximum dollar amount that may be issued per individual Task/Delivery Order is $1,000,000 per line item.

Minimum Order:   The minimum dollar amount that may be issued per individual Task/Delivery Order is one (1) eight-hour shift.

Cancellation of Orders:   Minimum two (2) hours notice prior to cancellation, no charge; facility will be billed for two (2) hours in the even of cancellation with less than two (2) hours notice.

Discount From List Prices or Statement of Net Price:   Prices shown herein are net ceiling prices, including malpractice insurance. Prices do not include travel costs.

Prompt Payment Terms:   2% 15 Days, Net 30 Days.
